Required Supplemental Information

Required Uploads: 1. A detailed description (no more than 2 double-spaced pages or appx 500 words) of the completed project or program including when the project was completed, how or why the nominee's activities are exceptional and how the nominee meets the award criteria. 2. A current board roster if the nomination is for a friends group or supporting nonprofit.
